- Annual report form requires that you list a participation floor for reporting outcomes on clients/participants. A participation floor is the minimum amount of the services a client/participant must have attended before you will report outcomes for the client/participant.
For most grantees, this floor is listed in your contract. For Round Three After-school and mentoring program grantees, you will need to set a participation floor for outcome tracking and list it in your annual report.
- You must include the number of clients/participants that you served who met your participation floor.
- List the outcomes specified in your contract.
- List the results for clients you served over the year. If clients/participants cycle through your program throughout the year, you will need to aggregate the results for all clients meeting the participation floor during the applicable period.
- Discuss the methods used for collecting and analyzing data.
- Answer questions regarding results.
